Search the ground
Pick up the leeches that spawn around Island 2 rather than fishing for them.
Use the current counter
Return to the relevant NPC when the on-screen quest counter is satisfied so the leeches become bait.
Catch Giant Piranha
Use the quest bait on the target and complete the encounter.
Turn in the trophy
Carry the recognizable boss trophy, not ordinary meat, to the NPC for the next coordinates.
